Dear Parents and Guardians,

The HPS Home Visit Program utilizes a schedule and structure that has been developed and proven to break down barriers that often exist between schools and the community, and to provide opportunities for the sharing of goals and expectations. From the first visit focusing on relationship building, the program solidifies and strengthens the relationship with families. It helps parents/guardians understand where their child is academically and provides tools to increase the parents’ capacity to assist their child in specific areas as needed.

As an HSI- Sugar Land teacher, we believe the importance of Home Visits. We would like to visit as many of our students’ families as we can and learn more about them. For this reason, we would like to have a short visit to your house at any available time for you.

Please select your available time from the schedule below. After we get this form we will call you back for the confirmation. All home visits will be after 4:00 pm. during the weekday and before 5:00 pm during the weekend.

If you have any questions or concerns, please feel free to email Mr. Kucuk by ali.kucuk@harmonytx.org.

Time: 6:00 PM – 7:00 PM
Location: HSI-Sugar Land Library (3rd Floor)

I would like to extend an invitation for you to participate in a financial aid information session/workshop. During this session, you and your Scholar will have an opportunity to review the financial aid process, participate in a demonstration for filling out necessary financial aid forms, and complete the Financial Aid Form for Student Application (FAFSA) with the help of a financial aid adviser.

• Scholar’s alien registration or permanent resident card if not a U.S. citizen
• Driver’s License of Scholar (if any)

• Financial information includes the following:
-W-2 Forms and other records of money earned in the previous year (if any- The IRS Data Retrieval Tool (https://studentaid.ed.gov/sa/fafsa/filling-out#irs-drt) can help you transfer your federal tax return information right into your FAFSA form.)
-Scholar’s Federal Income Tax Return from the previous year (if any)
-Parents Federal Income Tax Return (if Scholars are considered a dependent) W-2s, and other records of money earned.
-Scholar’s untaxed income records—(i.e. worker’s compensation, child support received)
